If you don’t have dental Insurance…
We offer an in office Saving Smiles program to all of our patients who do not have dental insurance.
Plan includes:
- 1 Initial Oral Evaluation
- 2 Cleanings
- 1 Periodic Exam
- 2 Emergency Exams
- 1 set of X-rays
The In-House plan will also give the patient a 10% discount on restorative and cosmetic dentistry as routinely performed in our office. This plan is not dental insurance and is exclusive to our office. The cost of this plan will be based on your periodontal assessment at the initial examination.
Premium due at time of contract.
The In-House plan will also give the patient a 10% discount on restorative and cosmetic dentistry as routinely performed in our office. Any services paid for using CareCredit® are not eligible for a 10% discount. This plan does not include coverage for procedures done by a specialist. This coverage will run for the calendar year and any unused services are non-refundable. All unused services will expire on December 31, of the contract year. Payment is due at the time of service. No Refunds. A 48 hour notification is required to cancel/change an appointment. Failure to provide this notice will result in a no call/no show fee added to your account balance.
